Membership Agreement
Purpose:
SERVN LOOKS BY F.C.M LODGE INC. is organized exclusively for social, charitable, educational, and informational purposes.
Organization Leadership:
Founder & Chairman: Patricia Sharon Muhammad
President of the Board: Patrell Brooks-Stallworth
Location: online
Membership Terms:
1. Membership in SERVN LOOKS BY F.C.M LODGE INC.
is open to individuals who support the mission and objectives of the organization.
2. All members agree to uphold the values and bylaws of the organization.
3. Membership requires the payment of a monthly fee of $50.00, due on the 1st of each month.
4. Membership may be suspended or terminated by the Board of Trustees for non-compliance or conduct detrimental to the organization.
Confidentiality:
Members shall not disclose any confidential information pertaining to the organization without prior written consent from the Board.
Acknowledgment:
By signing this agreement, the undersigned acknowledges and agrees to the terms of membership in SERVN LOOKS BY F.C.M LODGE INC.
